Public Health

The services provided by the Community Health program focuses on the health of the community as a whole and its individual members. The goals of the program are directed at three health care levels: health promotion, disease prevention and health maintenance.

Community Health Representatives (CHRs) and Community Health Workers (CHW’s) are well-trained medically guided Tribal or Native community-based health care providers who provide a variety of health services within our Tribal communities. Services include but are not limited to:

Home visits, transportation, and medication delivery
Case management
Health education and health promotion
Support and interpretation of health issues
